How to become a new patient:
We are sure you will find relief after meeting with Dr. Parker. We put an emphasis on improving the overall quality of your life, and we do this through courteous office visits, personalized treatment plans, and easy access to Dr. Parker through our PATIENT PORTAL
In some cases, Dr. Parker may require your medical records for review or a PCP referral before consideration of accepting as a new patient.
First visit expectation:

Expect a welcoming experience from our courteous staff

Expect Dr. Parker to spend quality time going through your health & medication history.

Expect a thorough medical exam to help pinpoint the pain generators.
What to bring to your initial visit:

Insurance card

ID
-
All medications currently taking in the bottles.

Any imaging or doctor reports pertaining to your current pain issue.
Referring providers:
If you are a Referring Provider and you would like to provide an opportunity for your patients to receive treatment from a pain specialist who will genuinely strive to either eliminate or lessen their pain issue, then please complete the following Referral Form
To ensure your patient is seen as quickly as possible, please fax the following documents with your referral request:

Recent Physician notes

Applicable radiology reports (MRI, X-Rays, etc.)
